New Student-Athletes (First Year & Transfers)
ALL NEW STUDENT-ATHLETES: Please see the tutorial below for full instructions and the checklist to follow to ensure that you complete all documentation required for participation in intercollegiate athletics at the University of Puget Sound. Delayed completion of these requirements may affect your ability to try-out or participate in any team activities.
*All new student-athletes must complete a pre-participation physical exam (PPE) using the University of Puget Sound Pre-Participation Physical Exam Form to be completed by a physician within 6 months of the first date of participation in the intercollegiate sport (NCAA Bylaw 17.1.5)

Returning Student-Athletes
ALL RETURNING STUDENT-ATHLETES Please see the tutorial below for full instructions and the checklist to follow to ensure that you complete all documentation required for participation in intercollegiate athletics at the University of Puget Sound. Delayed completion of these requirements may affect your ability to try-out or participate in any team activities.
*New for 2026 and beyond - all returning student-athletes are required to obtain a pre-participation physical exam (PPE) using the University of Puget Sound Pre-Participation Physical Exam Form to be completed by a physician within 6 months of the first date of participation in the intercollegiate sport.

MEDICAL FORMS NOW REQUIRE ELECTRONIC SIGNATURES TO BE VALID AND ACCEPTABLE
Depending on which Internet Browser you chose some configuration may be necessary. If you have trouble accessing the electronic signature boxes, please take the following steps based on your browser:
• Mozilla Firefox: Go to Tools>Options>Applications, the chose 'use Adobe Reader' for Portable Document Format (PDF)
• Google Chrome: Type 'chrome://plugins' into the address bar, then disable 'Chrome PDF Viewer'
• Internet Explorer and Safari do not require modifications
After completing these steps, please attempt to open a new window and access the necessary medical forms.
